Information for New Scholar-Athletes This medical packet is required for ALL new scholar-athletes coming to McM Athletics. It MUST be completed before participating in athletic practice and/or competition at the beginning of the current academic year.
Pre-Participation Physical

Mandatory for all new scholar-athletes.
Health Insurance Requirements This document outlines McM insurance requirements for scholar-athletes and what happens after a scholar-athlete is injured in regards to medical bills.
NCAA Banned Substance List An updated list of banned substances as issued by the NCAA
ADHD Reporting Form This form is required by NCAA to document a student-athlete's diagnosis of ADHD/ADD and must be filled out prior to the start of the academic year. This form is only required to be completed once during the student-athlete's time at McM.
NCAA Concussion Fact Sheet Required that all student-athletes read the NCAA Concussion Fact Sheet and acknowledge on Concussion Statement, which is part of the athlete packets.
NCAA Sickle Cell Fact Sheet
Sickle Cell Request Form
Required that all scholar-athletes provide sickle cell results. If born in Texas, here is the request form. All other states/countries please contact head athletic trainer.
